Approve a SOAP note (SLP)
Review and sign off on a completed SOAP note written by a CDA you supervise. Approval locks the note and makes the appointment payroll-eligible.
Before you start
- The CDA has marked the note complete (status includes ‘Soap Notes Completed’).
- You are set as the client’s supervising clinician.
Who can do this
The SLP assigned as the client’s current supervising clinician. Owners can override in exceptional cases.
Why: Clinical-compliance rule: CDAs cannot self-approve; an SLP must review before the note is finalized.
Step by step

From your dashboard, open the ‘Approve SOAP’ todo group. Each entry shows the client, appointment date, and the CDA who wrote the note.

Click an entry to open the approval modal. Review the full note and any template sections.

Optionally add a supervisor comment for the CDA, then click ‘Approve’.
The appointment status gains ‘Soap Notes Approved’, the note is marked finalized, and the CDA’s task is completed automatically.

If you need to change anything after approval, use ‘Edit as Supervisor’. Edits are tracked per-field and the CDA will get a ‘Review Supervisor Edits’ todo.
If something isn’t working
Tap a question to see the likely cause and the fix.
I don’t see an Approve button on a completed note.
Why: You aren’t the client’s current supervising clinician, even if you’re an SLP.
Fix: Ask the clinic manager to set the client’s supervising clinician. See ‘Why can’t I approve this SOAP?’.
The note was approved but the appointment still isn’t on payroll.
Why: Payroll readiness also requires the appointment to be invoiced and not in a ‘No Fee’ cancellation status.
Fix: See ‘Why isn’t this appointment on payroll?’.
I edited an approved note — will my CDA be notified?
Why: Yes. Field-level diffs are tracked and the CDA receives a ‘Review Supervisor Edits’ todo (unless you were also the treating clinician).
Fix: Let the CDA know the context in a comment when you edit so they understand what changed.
