Complete a SOAP note (CDA)

As a CDA, write and finalize the SOAP note for a treated appointment. Once you mark the note complete, your supervising SLP is automatically assigned an approval task.

Before you start

  • The appointment status includes ‘Treated’.
  • You are the treating clinician on the appointment.

Who can do this

The treating clinician on the appointment. Any clinician can save or update a draft; completing the note marks it ready for supervisor approval.

Why: Authorship is tied to the person who delivered the session so supervision and compliance trails stay accurate.

Step by step

    From your dashboard, open the ‘Soap’ todo group and click the appointment you want to document.

    Fill out the SOAP template. Save progress at any time to come back to it — the note will show ‘Soap Notes Saved’.

    You can reopen and edit any saved draft until you mark it complete.

    When the note is ready for your supervising SLP, click ‘Mark Complete’.

    The appointment will show ‘Soap Notes Completed’ and a task is created for your supervising SLP.

    Wait for your supervising SLP to approve. Once approved, the appointment becomes eligible for payroll and the note is locked to further edits.

    If your SLP edits the note after approval, you’ll get a ‘Review Supervisor Edits’ todo to acknowledge the changes.

If something isn’t working

Tap a question to see the likely cause and the fix.

I don’t see the appointment in my ‘Soap’ todos.

Why: The appointment status may not yet include ‘Treated’, or you are not the treating clinician.

Fix: Mark the appointment ‘Treated’ from the schedule, or ask the clinic manager to correct the treating-clinician assignment.

‘Mark Complete’ is disabled.

Why: Required template sections are empty, or the note has unsaved changes.

Fix: Save first, then complete any required sections before marking complete.

I need to fix my note after marking it complete.

Why: Once complete, only your supervising SLP, a supervisor, or the owner can edit it.

Fix: Ask your supervising SLP to either unapprove the note so you can edit, or to make the correction themselves.
