Review a form or bundle submission

Review submitted forms, download a PDF copy, compare submitted data with the client record, and apply only the changes that should become official.

Before you start

  • The client or family has submitted a public form or bundle link.
  • You have permission to view the submission for the clinic or client.

Who can do this

Owners, admins, clinic managers, and HR can review submissions broadly. Clinicians can view submissions for their own clients when enabled.

Step by step

    Open Forms > Submissions, or open the client's Forms tab and select the submission.

    Review each page of the response. Multi-page forms keep their section headings so the PDF and on-screen review match the client-facing form.

    Download the response PDF when a copy is needed for the client chart, insurer package, or audit trail.

    Open the client data diff. Compare submitted values with existing client values and choose the fields to apply.

    Apply the selected fields to the client. Leave conflicting or ambiguous fields unchanged until you confirm them with the client.

If something isn’t working

Tap a question to see the likely cause and the fix.

A client says they submitted, but I cannot find it.

Why: The bundle may be under another family member, the link may still be in progress, or the client may have submitted to another clinic.

Fix: Search the client and family records, then check the Forms dashboard activity. If needed, resend the bundle link.

A signature is missing from the PDF.

Why: The form field may not have been required, or the client resumed an old in-progress submission that predated the latest form requirement.

Fix: Request a new submission with the current approved form or send a standalone document for signing.
