Use the Forms command centre
Create forms, manage bundles, send forms to clients, review submissions, and understand the owner approval workflow for published forms.
Before you start
- You have access to Forms from the sidebar, or you are on a client's Forms tab.
- For public submissions, the clinic contact details and branding should be correct.
Who can do this
Owners, admins, clinic managers, and HR users can access the Forms command centre. Clinicians access client-specific forms and filtered submissions from the client profile.
Why: Form creation and bundle configuration affect client-facing intake and consent workflows, so publishing is controlled by role.
Step by step

Open Forms from the sidebar. Start on Dashboard for form metrics, recent activity, and shortcuts.

Use Form Builder to create or edit a form. Draft forms can be submitted for approval when they are ready for owner review.

Owners approve the form or request changes with a comment. Approved forms stay usable while later revisions move through a new draft and approval cycle.

Use Bundles to group approved forms into client-facing packets. Bundles can be sent from the command centre or directly from a client's Forms tab.

Use Submissions to review completed packets, download response PDFs, compare submitted client data, and apply approved changes to the client record.
If something isn’t working

Why can I not add a form to a bundle?
Why: Bundles accept approved forms and legacy active forms. A draft or pending form is not available for client-facing use.
Fix: Submit the form for approval, wait for owner approval, then add it to the bundle.
Why can I edit an approved form only as a revision?
Why: Approved forms may already be in active bundles. Simone keeps the published version stable and clones a new draft revision for changes.
Fix: Use Create Revision, make your edits, and submit the revision for approval.
