Create a treatment plan

Create a goal-based treatment plan for a client, link it to the SLP and clinician team, and make goals available for progress-summary reporting.

Before you start

  • The client exists in Simone.
  • You have the assessment findings, current communication status, goals, target behaviours, and progress indicators ready.

Who can do this

SLPs and clinical leadership can create treatment plans. Admin and clinic manager access depends on clinic configuration and is usually for support or correction.

Why: Treatment plans are clinical records and feed progress-summary goal tracking.

Step by step

    Open the client record, then open Treatment Plans or the linked case workspace.

    Create a new treatment plan. Confirm the client, date, SLP, clinician team, school or grade details, and current communication status.

  3. 3

    Add each goal with target behaviour, progress indicators, and action steps. Keep goals measurable so later progress summaries can reuse them.

    Save the plan. It remains in the client's treatment plan history and the latest goals can populate the progress summary workflow.

    Download or generate the treatment plan PDF when the client chart, insurer, or school documentation needs a copy.

If something isn’t working

Tap a question to see the likely cause and the fix.

Goals are not appearing in a progress summary.

Why: The progress summary loads from the latest linked treatment plan for the client or case. The plan may not be linked or may not be the latest record.

Fix: Open the client's Treatment Plans tab, confirm the newest plan has the expected goals, and check the case link.

I cannot edit or delete a plan.

Why: Some edits are limited to the creator or an admin-level role because treatment plans are clinical records.

Fix: Ask the creating SLP or an admin to make the correction, preserving the reason for the change in notes when appropriate.
